• To be covered by FMLA (The Family and Medical Leave Act) there are two criteria for our employees, the first an employee must have worked for the School District of Ashland for at least 12 months; and second is least 1,250 hours of service has been performed for the School District of Ashland during the 12 month period immediately preceding the leave. 

    Please fill out the FMLA leave form.  If you have any questions please call Payroll Benefits X6008 (Laura Graf). 

    If needed we will ask that you complete the second form, Health Care Provider Certification.  Employees will not be asked for this form to be completed if leave is for Maternity Leave under FMLA.  

    Also, employee's requesting long term leave must also complete the Request for Long Term Substitute form.
