The State of Wisconsin's Student Immunization Law requires students to be immunized against specific contagious illnesses within 30 school days of beginning school. Please submit your child's immunization record to the school health office. You may provide a printed copy from the Wisconsin Immunization Registry (WIR) or complete the Student Immunization Record form. Waivers are available for religious, personal, or medical reasons and can be indicated on the Student Immunization Record form (link above). Medical waivers require the signature of a licensed medical provider.
