Immunizations

The State of Wisconsin's Student Immunization Law requires students to be immunized against specific contagious illnesses within 30 school days of beginning school. Please submit your child's immunization record to the school health office. You may provide a printed copy from the Wisconsin Immunization Registry (WIR) or complete the Student Immunization Record form.

Waivers are available for religious, personal, or medical reasons and can be indicated on this Student Immunization Record Form. Medical waivers require the signature of a licensed medical provider.

 

Flu Vaccine

In the United States, Seasonal Influenza typically occurs between the months of October and May. This illness can cause severe symptoms and complications in some people. A yearly vaccine is recommended for everyone six months of age and older (consult with your primary health provider for possible contraindications).

Flu Vaccine Protects

The Ashland County Health Department conducts an annual Influenza Clinic for our students. The Influenza Clinic typically takes place in October. Permission slips are sent home with students prior to the Influenza Clinic.
